LPN

Provide skilled nursing care/treatment to client per Physician orders and Plan of Care

Accurately document nursing actions of all care given, individual's health status, and communicate with family and physicians

Document progress and outcomes for established  goals and informs team of any changes to client's medical condition or needs

Medication administration and management as needed

Follow all policies and procedures according to operation, state and regulatory standards

Must have a valid driver's license, car and valid car insurance, Current LPN license
